Care Manager

Qualifications:

Minimum of a BA/BS in human services field; experience with elderly a plus; working knowledge of Microsoft applications preferred. Vehicle and valid driver’s license needed. E.O.E.

Responsibilities:

The Agency is a subcontractor for the Allegheny County Area Agency on Aging. Responsibilities include providing care management services to homebound older adults and disabled individuals in the community, in the Eastern part of Allegheny County; performing home visits; developing care plans; arranging and coordinating supportive services; providing on-going monitoring and reassessments; acting as a liaison and advocate for consumers.

Salary:

$24,000/yr. plus incentives and benefits (Salary commensurate with care management experience)

Working Conditions:

Travel is required throughout the service area. The position requires visits to various settings in the community which include private residences.
